Sarnano borders the following municipalities: Amandola, Bolognola, Fiastra, Gualdo, Montefortino, San Ginesio. It is one of I Borghi più belli d'Italia ("The most beautiful villages of Italy").

The medieval castle of Gualdo belongs to the Bonifazi of Monte San Martino then to the Brunforte. After a brief period of peace feuds started with the Varano and then the Sforza. Parts of the castle walls and towers remain visible in the 21st century.

The Abbey of San Biagio was a former Roman Catholic monastery located in a rural site in the village of Piobbico, within the limits of the commune of Sarnano, in the province of Macerata, region of Marche, Italy. Of the abbey, only the church, dedicated to St Blaise, and a small monastic apartment remain.

The Monte San Martino Trust was founded in 1989 by J. Keith Killby, a former prisoner of war at Servigliano nearby, together with other veterans of the Second World War. The Trust awards English-language study bursaries to Italians, aged 18 to 25, in recognition of the courage and sacrifice of the Italian country people who rescued thousands of escaping Allied PoWs after the Armistice in 1943.
